| Appointments: We offer the convenience of expanded patient hours with after 5:00 appointments as well as one Saturday morning each month. We make every effort to honor all time commitments and request that you extend the same courtesy to us. If you are unable to keep an appointment, please notify us within 24 hours. We reserve the right to charge for appointments that are cancelled or not kept, unless 24 hours notice is given. |

| Emergency Care: If an emergency arises, please call us as early in the day as possible, and we will do our best to respond to your problem promptly. If after office hours, our office is equipped with an answering machine that we regularly review. |

| Recare visits: We hope you share in our belief that regular preventive dental health care is a sound investment. While the responsibility for returning for this treatment rests primarily with you, we will provide the service of sending you a written reminder when it is time to return. |

| Financial Arrangements: We have several methods of payment that help make treatment affordable. These options will meet the needs of most patients in our practice, and we are pleased to offer them as a service to you. |
| Payment by the appointment - This option lets you spread out the payments according to your treatment plan. |
| Mastercard, Visa, Discover - This option will let you know how much your monthly payments will be so that you can budget your payments comfortably. |
| Insurance on Assignment - We will file your insurance as a service to you and will do our very best to maximize your benefits. We accept assignment of benefits to lower your immediate "out of pocket" expenditures. We ask that you take care of your estimated portion of payment at the time of the service. We will file your insurance at the same time. |
| Important note about insurance: Please remember that no insurance company attempts to cover all dental costs. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge based on the contract with your employer. It is your responsibility to pay any deductible amount, co-payment, or any other balance not paid for by your insurance company. |
| Financing through our financing partner, CareCredit - Upon approved credit, CareCredit is a financing program that lets you make small monthly payments spreading them out over time. Finance your entire treatment, or the balance of your insurance portion. It is very easy to apply for, and approval time is just minutes. |
| Senior discount - A courtesy discount is given to patients age 65 and better. |
